This is a modern wire tightener ,, I suspect the image above is an older version.......
used on farms to put up fences and also used by grape growers to set up vine trellises

i see barbed wire in your photo ...... it even says on the photo upload ......... tightener-on-barbed-wire-fence ........

i " think " the tool is used for removing wood rims from a old stage coach .......

I think it's a tool used during child birth to ease delivery (a spreader) LOL !!
