Top Level IPTV

Is there a top level iptv provider? like i assume many are selling and reselling and transcoding the same streams... many providers seem to have the same streams and the same playlists but at varying prices. Obviously each iptv provider doesn't have a thousand cable boxes on the go...what are some that are higher up the totem pole?
