Special Upgrade Offer for Verizon customers who have recently purchased a new phone.

Here's how the program works:
To be eligible for the Special Upgrade Offer you must be a current Verizon Wireless customer who purchased and activated a new smartphone, feature phone or certified pre-owned phone between November 26, 2010 and January 10, 2011.
Purchase or place a confirmed order for a new iPhone 4 at full retail price from a Verizon Wireless retail location (store or online) by February 28, 2011.
After you have received and activated your iPhone 4, visit verizonwireless.com/promoexchange to complete and submit your debit card request.
Once your submission has been validated, we will send a prepaid mailer for the return of your eligible smartphone, feature phone or certified pre-owned phone.
Return your eligible phone in the prepaid mailer within 30 days of activation of your iPhone 4, and we'll send you a VisaŽ debit card within 4 to 6 weeks.
Eligible customers will receive a $200 Visa debit card for smartphones, or a $75 Visa debit card for feature phones and certified pre-owned phones.
Note: Offer valid only for existing Verizon Wireless customers on accounts with 5 lines or less. Customers must have purchased and activated a new Verizon Wireless smartphone or feature phone or a Verizon Wireless certified pre-owned phone between November 26, 2010 and Janurary 10, 2011. To qualify for debit card, iPhone 4 must be activated with the same mobile telephone number as the phone to be exchanged. If you return or exchange your iPhone 4, you will no longer be eligible for debit card.
